Responsibilities

• Evaluate patients and create individualized plans of care
• Providing 1 to 1 patient care
• Deliver hands-on, patient centered treatment and education
• Monitor progress and adjust care plans to achieve meaningful outcomes

Qualifications

• Licensed Physical Therapist (or ability to obtain licensure in the state of Wisconsin)
• Passion for evidence-based, patient centered care

About Compensation: $75K -$100K per year + productivity bonuses, with additional compensation opportunities for certifications and location differentials.
