About the Role:

As a Physical Therapist (Per Diem) at Spring Creek Healthcare Center, you will have the opportunity to work with a diverse patient population, providing physical therapy services to help them achieve their maximum level of function and independence. Your expertise will be invaluable in developing and implementing individualized treatment plans, conducting evaluations and assessments, and collaborating with our multidisciplinary team to ensure comprehensive care.

Responsibilities:

• Conduct initial and ongoing evaluations of patients/residents to establish and update treatment plans
• Develop and implement individualized treatment plans to achieve patient/resident goals
• Provide physical therapy services, including but not limited to: exercise, modalities, and manual therapy
• Collaborate with other healthcare professionals to ensure comprehensive care
• Communicate with patients/residents, families, and other healthcare professionals to ensure effective care coordination
• Maintain accurate and detailed records of patient/resident progress and treatment plans
