What you’ll do

• Review medical history and assess each resident’s condition and needs
• Evaluate a resident’s ability to perform daily tasks and movements
• Build individualized treatment plans with clear, achievable goals
• Guide residents through exercises that improve mobility and relieve pain
• Recommend and train residents on mobility equipment such as walkers and wheelchairs
• Educate families on how to support recovery at home
• Track progress against goals and prepare reports for providers and insurers
• Work with physicians, nurses, therapists, and social workers as part of the care team

What you need

• Active Indiana professional license as a Physical Therapist
• Graduate of an accredited Physical Therapy program
• Compassionate, patient, and able to think critically under pressure
• Strong communication skills with residents, families, and staff
• Long-term care experience preferred
