About the Role:

As a Physical Therapist at West Park Rehabilitation and Nursing Center, you will be responsible for:

• Designing and implementing comprehensive rehabilitation plans for our residents, focusing on improving functional abilities and overall well-being
• Conducting thorough evaluations and assessments to identify physical therapy needs
• Developing and implementing individualized treatment plans to address resident goals and outcomes
• Collaborating with multidisciplinary teams to ensure residents receive coordinated, patient-centered care
• Providing education and training to residents, families, and caregivers on physical therapy techniques and exercises
• Documenting treatment plans, progress notes, and outcomes in a timely and accurate manner
