Position Responsibilities

• Provide skilled physical therapy interventions in patients' homes under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans, including therapeutic exercise, gait training, balance activities, transfer training, and functional mobility.
• Monitor patient progress and communicate changes in condition to the supervising Physical Therapist.
• Complete timely and accurate clinical documentation.
• Coordinate with the supervising PT and other members of the care team to support quality patient outcomes.

Qualifications

• Active Florida Physical Therapist Assistant license in good standing.
• Graduation from an accredited PTA program.
• Current CPR/BLS certification.
• Reliable transportation and willingness to travel to patients' homes.
• Strong communication, organization, and clinical skills.
• Home health experience preferred, but motivated clinicians with other relevant experience are encouraged to apply.
