Responsibilities:

• Evaluate patients and develop individualized care plans
• Provide hands-on therapy and education in the home
• Communicate with physicians and care team members
• Document progress and update treatment plans as needed

Qualifications:

• Licensed Physical Therapist (PT) in Florida
• Graduate of an accredited PT program
• Home health experience preferred (new grads welcome)
• Reliable transportation and valid driver’s license
