What they need
Job Overview
We’re looking for a board-certified physician in internal medicine or family medicine to provide primary care across our facility and home-based patients. You’ll split your time between partner facilities and house calls, lead each patient’s care plan and work alongside our NPs and support staff. If you value relationship-based medicine and want time to actually know your patients, this role is for you.
Responsibilities
- See patients in independent living, assisted living and other care facilities, and in patients’ homes: wellness visits, chronic care follow-ups and acute visits.
- Manage common chronic conditions in older adults, including hypertension, diabetes, COPD, CHF, dementia and fall risk.
- Order and review labs, mobile X-rays and other diagnostic tests, and coordinate referrals to specialists.
- Certify and sign home health orders and plans of care, and coordinate with home health and hospice agencies when patients need more support.
- Communicate with facility staff, caregivers and families on changes in condition and care plans.
- Serve as the collaborating physician for our nurse practitioners, including prescriptive authority agreements under Texas rules, chart review and clinical guidance.
- Lead medication reviews and reduce unnecessary prescriptions for patients taking many medications.
- Hold advance care planning and goals-of-care conversations with patients and families.
- Document visits accurately and on time in our EHR, with correct ICD-10 and CPT coding, including home and domiciliary visit codes.
- Follow HIPAA requirements and protect patient privacy and safety.
Qualifications
- MD or DO from an accredited school.
- Active, unrestricted Texas medical license, plus DEA and Texas DPS registrations (or able to obtain them before your start date).
- Board certified or board eligible in internal medicine or family medicine.
- Experience in primary care, geriatrics, house calls or post-acute/facility medicine is strongly preferred.
- Comfortable collaborating with and supervising nurse practitioners.
- Familiar with Medicare, annual wellness visits, home health certification and hospice referrals.
- Comfortable with EHR documentation and ICD-10/CPT coding.
- Valid driver’s license, reliable transportation and auto insurance for travel between facilities and homes.
- Warm, patient communicator with older adults, families and facility staff.
