This role is with a Federally Qualified Health Center (FQHC) serving diverse communities in Santa Clara County. The position offers outpatient primary care services, typically 32 to 40 hours per week.

Responsibilities

• Provide patient-centered outpatient primary care for all ages (no obstetrics)
• Conduct physical exams, interpret diagnostics, and develop treatment plans
• Collaborate with multidisciplinary teams, including behavioral health
• Manage chronic disease through education and self-care support
• Coordinate specialty and inpatient referrals
• Complete various patient and medical forms
• Participate in quality improvement, peer reviews, and clinical audits
• Participate rotating on-call duties for afterhours support
• Engage in community health activities and events

Qualifications

• MD or DO from an accredited institution
• Board certified in Family Medicine, Internal Medicine, or related specialty
• Current unrestricted California medical license
• Active unrestricted DEA License
• Minimum 2 years of primary care clinical experience
• Current Basic Life Support (BLS) certification
• Ability to manage a full-time patient panel (~1,500 patients)
• Passion for serving underserved, multicultural communities
• Strong team collaboration and communication skills
• English proficiency required; additional languages a plus
