About the role

You will work as part of an integrated multidisciplinary Allied Health team delivering person-centred services across aged care, restorative and community environments.

Key responsibilities

• Mobility & transfer assessment
• Balance & falls risk assessments
• Multiple outcome measure administration i.e DEMMI, SPPB for tracking functional progress
• Individualised rehabilitation with graded loading, falls-prevention strategies and group exercise class facilitation
• Post falls review
• Working collaboratively in a multidisciplinary team, including attending multidisciplinary case conferences alongside doctors, pharmacists and allied health staff, contributing to discharge planning, care pathways and clinical decision-making
• Manual handling assessments and care staff education
• Reablement and restorative interventions

About you

• Work experience in community aged care
• Experience working with TCP clients is desirable but not essential
• Passionate about older people and quality care
• Practical, collaborative and compassionate
• Strong communicators
• Open to innovation and service improvement
• Registered with AHPRA
