Brockwood Medical Practice is looking for an enthusiastic and motivated Practice Nurse to join our expanding multidisciplinary team.

This is an exciting opportunity for either a newly qualified or experienced nurse who is passionate about delivering high-quality, patient-centred care in general practice and keen to develop their clinical knowledge and skills.

Job responsibilities:

*Long-Term Condition Management

Undertake assessment, monitoring and review of patients with long-term conditions including Diabetes, Respiratory Conditions, Cardiovascular disease and hypertension

*Vaccinations and Immunisations

*Cervical Screening

*Wound Care

*ECGs and Cardiovascular monitoring

*INR monitoring

*Phlebotomy and Clinical Investigations

*Women's Health

*Men's Health

*NHS Health checks

*Dementia Screening

*Vitamin B12 injections

*Health Promotion and Prevention

*Clinical Room and Stock Management

*Administrative Responsibilities
