What they need
Title: Practice Transformation Specialist
Office Location: Remote (California); Southern California preferred
Division: CMA Physician Services Organization (CMA PSO)
Exemption Status: Exempt
Reports to: Senior Program Manager, Practice Transformation
Salary range: $80,000 to $85,000 Annually
POSITION SUMMARY:
CMA PSO partners with healthcare organizations serving diverse and often underserved communities across California to strengthen primary care, improve quality outcomes, and support sustainable transformation in value-based care environments. The CMA-PSO is looking for one (1) Practice Transformation Specialist to help facilitate transformative change within independent physician practices, IPAs, and Federally Qualified Health Centers (FQHCs) participating in large-scale Medi-Cal quality improvement and value-based care initiatives. Working with guidance from senior Practice Transformation team members, the position builds productive relationships with clinic teams, identifies day-to-day operational barriers, and helps translate improvement plans into practical workflows and sustained action. The role manages multiple assigned practices and project responsibilities; supports meetings, learning collaboratives, and peer-sharing activities; reviews quality and operational data; and helps develop project-specific resources. Success requires consistent follow-up, clear documentation, strong collaboration, and measurable progress from planning through implementation. Within the first six months, success means becoming a dependable partner to assigned practices, maintaining consistent engagement and documentation, and helping teams move improvement work from planning into implementation. The strongest candidates bring curiosity, organization, relationship-building skills, and comfort using data and practical tools to solve day-to-day operational challenges.
DUTIES/RESPONSIBILITIES:
Manage assigned practices, health centers, and project responsibilities with guidance from senior team members, maintaining productive relationships, organized workplans, and timely follow-up.
Partner with physicians, practice leaders, and staff to implement workflow and quality improvement interventions related to preventive screenings, care-gap closure, Medi-Cal HEDIS/MCAS performance, encounter data, population health, and value-based care readiness.
Coordinate and facilitate focused provider and staff working sessions, learning collaboratives, and peer-sharing activities; prepare agendas, materials, notes, and action items as needed.
Review quality and operational data with senior team members, identify patterns and barriers, and translate findings into clear action steps for participating organizations.
Support improvements to clinical documentation, coding, reporting, population health tool use, and other processes that affect quality performance and operational efficiency.
Develop project-specific resources with guidance from project leaders, which may include templates, tip sheets, worksheets, presentations, workflow tools, and facilitated learning materials.
Track practice engagement, project milestones, barriers, progress, and follow-up needs across multiple assignments; maintain accurate and timely project documentation.
Identify implementation barriers, elevate concerns appropriately, and contribute practical solutions that help organizations move from planning into implementation and sustain new workflows.
Collaborate effectively with internal project teams, clients, health plans, IPAs, and provider organizations, and communicate progress, risks, and resource needs clearly.
Contribute to measurable improvement in assigned quality, workflow, care-gap, and implementation goals while developing deeper expertise in practice transformation and value-based care.
Other duties as assigned.
CONTACTS:
Independent practice and health center physicians and other providers; practice managers; front- and back-office staff; quality, operations, and care coordination teams; health plan and IPA partners; client project staff and leadership; CMA PSO management and supervisory staff; CMA colleagues; and C-suite or executive leaders as appropriate.
KNOWLEDGE/EXPERIENCE/EDUCATION:
Three or more years of experience supporting healthcare quality improvement, practice transformation, practice operations, population health, or related initiatives.
Experience working with physician practices, FQHCs, IPAs, health plans, health systems, or similar healthcare organizations.
Familiarity with Medi-Cal, HEDIS, MCAS, value-based care, encounter data, quality incentive programs, or population health operations.
Experience supporting workflow changes or improvement activities across clinical and operational teams.
Strong relationship-building, meeting coordination, written communication, verbal communication, and developing facilitation skills.
Ability to review quality or operational data, identify patterns, and help translate findings into action; working knowledge of Excel and experience using reports or data tools.
Ability to manage multiple assignments, deadlines, and provider organizations with appropriate guidance while maintaining organization, quality, and follow-through.
Ability to work independently on assigned tasks, ask for guidance when needed, and collaborate effectively across teams.
Familiarity with quality incentive programs, electronic medical records, population health platforms, project or workflow tools, and reporting or data visualization tools such as Power BI or Tableau is preferred.
Familiarity with emerging AI-enabled workflow tools and interest in using technology to improve efficiency and insights is preferred.
Bachelor’s degree in healthcare administration, public health, business administration, or a related field, or an equivalent combination of education and relevant experience, is preferred.
