Make a meaningful difference in people's lives

Are you an experienced health, rehabilitation or social care professional with strong leadership, organisational and communication skills? Do you have a passion for supporting individuals with acquired brain injury (ABI), disabilities or complex care needs to achieve the best possible outcomes?

Education and Case Management Services (ECMS) is looking for a dedicated and proactive Practitioner Case Manager to join our team in Gateshead.

This is an exciting opportunity for someone who enjoys coordinating multidisciplinary teams, supporting and developing care staff, and helping to ensure that clients receive high-quality, person-centred care and support.

Working with the guidance and supervision of a Case Manager or Senior Case Manager, you will play a key role in the day-to-day coordination of client services, supporting families, monitoring care delivery and ensuring that care and support plans are implemented effectively.

About the role

As Practitioner Case Manager, you will take a leading role in coordinating care and support services, primarily within established, non-litigation cases. You may also support work on litigation cases where appropriate, under the guidance and supervision of a Case Manager or Senior Case Manager. You will work closely with clients, families, support workers, therapists, multidisciplinary professionals, Deputies and legal representatives to promote safe, effective and consistent care.

Your responsibilities will include:

  • Care coordination: Coordinate multidisciplinary teams and support worker teams, ensuring effective communication and collaborative working.
  • Meetings and documentation: Chair multidisciplinary team (MDT) meetings, prepare accurate meeting notes and ensure actions are followed up.
  • Care planning: Draft, review and update person-centred care and support plans in line with relevant Care Quality Commission (CQC) guidance and organisational procedures, for approval by the appropriate senior manager.
  • Quality and compliance: Monitor support worker compliance with care plans, ECMS policies and procedures, escalating concerns where necessary.
  • Staff supervision and development: Lead support worker supervisions, provide guidance and training, and ensure staff understand and follow care plans, agreed goals and objectives, and recording requirements.
  • Clinical decision-making: Support clinical decision-making and problem-solving within your competence and under the supervision of a Case Manager.
  • Family liaison: Develop positive working relationships with clients' families, providing appropriate guidance, updates and support.
  • Service costing: Prepare cost estimates for care and support services for senior approval and subsequent submission to Deputies and legal teams.
  • CQC registration: Contribute to and support the CQC registration process, working alongside senior colleagues.
  • Administrative coordination: Take a leading role in directing the ECMS Administrator on support worker-related administrative and HR processes.
  • Stakeholder communication: Liaise with Deputies and their representatives, providing timely updates and addressing issues as required.
  • Staffing and HR support: Work with relevant managers and administrative colleagues to help resolve support worker HR issues, referring matters to the appropriate person.
  • Leadership and representation: Deputise for the Case Manager or Senior Case Manager when required, representing ECMS professionally.
  • Risk management and problem-solving: Anticipate potential difficulties, respond proactively to emerging issues and escalate concerns promptly to the appropriate senior colleague.

About you

We are looking for someone who is organised, dependable and confident in taking responsibility for coordinating people and services. You will be able to work independently with minimal supervision while recognising when to seek guidance or escalate concerns.

You will ideally have:

  • Good working knowledge of acquired brain injury (ABI), disability services or related care provision.
  • Experience of rehabilitation, therapy and/or social care services.
  • Proven experience supervising, directing or instructing others.
  • Experience working with support workers and coordinating their activities.
  • Strong planning, time management and organisational skills.
  • Excellent written and verbal communication skills, including active listening and the ability to communicate effectively with a range of stakeholders.
  • Good IT skills and an understanding of accurate record-keeping requirements.
  • The ability to understand, analyse and synthesise complex information.
  • A collaborative approach and the ability to work effectively as part of a multidisciplinary team.
  • A full, clean driving licence and the ability to drive.

Desirable: Knowledge and understanding of CQC standards and requirements.

What we are looking for

The ideal candidate will be a proactive and compassionate professional who can balance effective team leadership with a person-centred approach. You will be confident supporting staff, building relationships with families and professionals, maintaining high standards of documentation and identifying opportunities to improve service delivery.

You will understand the importance of confidentiality, safeguarding, professional boundaries and working within your competence, seeking senior guidance when appropriate.

This position is particularly suited to someone with experience in case management, rehabilitation, community care, complex care coordination or disability services who is ready to take the next step in their professional development.

Why join ECMS?

You will work within a supportive management structure, with opportunities to develop your leadership and case coordination skills while contributing to positive outcomes for clients.
