Responsibilities

• Conduct thorough patient assessments including vital signs, physical examinations, and medical history reviews using EMR/EHR systems such as Epic and Athenahealth.
• Prescribe medications in accordance with DEA regulations and develop individualized treatment plans for acute and chronic conditions.
• Perform procedures such as suturing, IV insertions, phlebotomy, catheterization, and sterile processing techniques.
• Manage patient care across various settings including outpatient clinics, telehealth platforms, urgent care, and nursing homes.
• Collaborate with healthcare professionals across specialties like gastroenterology, infectious disease care, behavioral health, geriatrics, and hospice & palliative medicine to ensure comprehensive patient management.
• Provide health coaching, patient education on disease prevention, medication administration, nutrition, and lifestyle modifications.
• Document all clinical encounters accurately in EMR/EHR systems while maintaining HIPAA compliance and medical records integrity.
• Participate in clinical research initiatives or quality improvement projects aimed at enhancing healthcare delivery standards.

Qualifications

• Valid Nurse Practitioner license with current certification to practice independently in the state.
• Proven experience working with diverse populations including pediatrics, geriatrics, behavioral health, or hospice & palliative care.
• Strong proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or similar platforms.
• Skilled in performing clinical procedures including suturing, IV infusion/insertion, catheter care, and sterile techniques.
• Knowledge of pharmacology for prescribing medications within scope of practice; familiarity with CPT coding for billing purposes.
• Critical care experience preferred; ICU or emergency room experience is highly valued.
• Experience working in hospital medicine or Level I/II trauma centers is a plus; familiarity with telehealth services is advantageous.
• Excellent client communication skills with a focus on patient-centered care; ability to collaborate effectively with multidisciplinary teams.
• Additional skills such as gastrology procedures (e.g., gastric lavage), spinal tap (lumbar puncture), sonography or medical imaging are beneficial but not required.

Pay: $96,300.00 - $117,000.00 per year
