Private Duty LPN

Role: Home and Community Based Services Licensed Practical Nurse (LPN)

Responsibilities

• Provide direct nursing care to acutely and chronically ill patients in their homes, following physician orders and within the scope of practice.
• Document patient visits thoroughly in electronic medical records and electronic visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
• Adapt to diverse home environments and treat all patients with respect regardless of background.

Qualifications & Skills

• Current, unrestricted LPN licensure in the practice state.
• Reliable transportation or public transit access, and a valid driver’s license.
• Preferred: CPR certification, ability to work flexible hours, manage multiple tasks independently, and strong communication and organizational skills.

Key Duties: Deliver high-quality clinical services, ensure accurate documentation, participate in care coordination, and provide compassionate patient care in home settings under supervision.
