Private Duty LVN

Hello my name is Laura, I am the mother of Alex. He is sweet, loving, socially aware, non-verbal young man, 30 years old. He is small and thin, has cerebral palsy, quadriplegic has a Hoyer lift. He is trach, ventilator, g-tube dependent and requires bathing and diapering, feeding, medication and breathing treatments. Total care ADLs and includes lifting and repositioning

Currently seeking daytime shifts, weekdays and weekends. ALSO seeking per diem for days and night shifts

Currently seeking nurses who is looking for a long term relationship with Alex and his family. Home health, trach, ventilator experience, preferable to have 2+ years of subacute care in facitlity or ICU. Someone who enjoys a one on one nursing relationship with a special person and possesses positive attitude and disposition. Alex reponds and thrives to positivity.

Responsibilities

• Provide comprehensive patient care in a home hospital setting, including monitoring vital signs, communicating with doctors, and administering medications.
• Assist with tracheotomy, tube feeding, and ventilator management as needed.
• Conduct patient assessments to evaluate health status.
• Ensure compliance with HIPAA regulations and maintain accurate EHR documentation.
• Collaborate with healthcare teams to coordinate care for patients with diverse needs, including those with developmental disabilities.

Job Types: Full-time, Part-time, Contract, Per diem

Pay: $45.00 - $50.00 per hour
