Responsibilities

• Perform thorough patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health records (EHR) systems such as Epic and Cerner.
• Administer medications, injections, IV infusions, and catheter care following strict aseptic techniques and safety protocols.
• Conduct diagnostic evaluations, specimen collection and processing, and assist with medical imaging procedures as needed.
• Manage airway maintenance, ventilator support, and acute pain management for patients in various settings including hospitals, skilled nursing facilities, and home care environments.
• Provide specialized care for patients with complex conditions such as Alzheimer’s disease, dementia, infectious diseases, or those requiring hospice or palliative care.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices in geriatrics and pediatrics.
• Ensure compliance with HIPAA confidentiality policies, medical documentation standards, ICD coding (ICD-9/ICD-10), and other regulatory requirements.
• Utilize triage skills to prioritize patient needs efficiently in urgent or emergency situations within emergency rooms or urgent care settings.
• Support patient education on medication administration, nutrition, wound care, and health coaching to promote optimal recovery and well-being.

Qualifications

• Valid RN license in good standing with current state licensure requirements.
• Proven experience working across multiple healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, or home & community care.
• Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or similar platforms.
• Perform daily wellness checks and document findings
• Monitor participants for changes in condition
• Provide medication assistance per agency policy
• Communicate concerns to families, providers, and management
• Maintain compliance with all Texas HHS Adult Day Care regulations
• Support staff in maintaining a safe, positive environment
• Knowledge of ICD coding (ICD-9/ICD-10), medical terminology, anatomy and physiology relevant to diverse patient populations.
• Ability to perform physical examinations and assessments accurately while maintaining clinical confidentiality policies.
• Excellent communication skills for caregiver interaction, client education and interdisciplinary collaboration.
• Experience working with individuals with disabilities or developmental delays is a plus.

Pay: $20.00 - $22.00 per hour
