Job Summary:

We are seeking a Per Diem Licensed Practical Nurse (LPN) to join our team at TruHeart Home Health - Norman. Essential Functions:

• Provide prescribed nursing care services to patients in the home care setting, following a Plan of Care (POC) and according to patient needs
• Monitor patient condition, report abnormalities, and make necessary adjustments to care plan
• Assist patients with daily living activities, such as bathing, dressing, and grooming
• Administer medications and treatments as prescribed by physicians
• Maintain accurate and concise records of patient care and status
• Collaborate with other healthcare professionals to ensure comprehensive care
• Participate in on-call rotation as required
• Perform selected skills actions, such as wound care and IV maintenance
• Identify and address patient needs and problems, and report concerns to supervising nurse
• Maintain a professional and respectful attitude, and promote teamwork and harmony with patients, family, and staff

Responsibilities:

• Demonstrate knowledge and competency in performing baseline nursing skills and procedures
• Implement nursing care plans and make necessary adjustments
• Participate in patient care conferences and team meetings
• Maintain a safe and efficient work environment
• Follow all agency policies and procedures
• Participate in quality improvement initiatives and maintain accurate records

Additional Requirements:

• Must pass a criminal background check and drug test
• Must have a valid Oklahoma LPN license, CPR certification, and driver's license
• Must have a minimum of 1 year of experience in home health or hospital setting
• Must have completed competency assessments
• Must have a strong understanding of HIPAA regulations and maintain confidentiality

Work Attire:

• Scrubs

Environmental Conditions:

• Moderate noise level; inherent potential for exposure to bloodborne pathogens, moderate stress, and emotional demands

Physical Requirements:

• Lifts and Carries (occasionally 3 hours or less daily)
