Program Coordinator

Job Duties

Description We are seeking a full time Program Coordinator for our location in Coon Rapids!

Program Coordinator Responsibilities Assist the Program Manager in ensuring that the tasks on resident care plans are completed

Assist with schedule changes

Ensure completed documentation

Provide quality personal care services and health-related services

Program Coordinator Qualifications A two-year-degree in a social services area of study or an equivalent combination of experience and education

Minimum of two years of experience working with adults with physical disabilities and/or traumatic brain injuries in a rehabilitation or transitional long-term-care setting

Ability to model excellent work behaviors and team-oriented problem solving

Strong problem-solving skills and the ability to self-motivate and effectively manage time and shift priorities

Ability to accompany and/or transport residents within the community
