What they need
Position Summary:
The Program Coordinator will be responsible for overseeing the administration and operational efficiency of Missouri's Medicaid programs related to consumer-directed care, structured family caregiver and in-home services. The manager will ensure the delivery of high-quality, cost-effective services to Medicaid recipients who require assistance with activities of daily living (ADLs) while maintaining independence in their homes. The role will involve program oversight, compliance, stakeholder engagement, staff management, and continuous improvement to support recipients in accessing care and living independently.
Key Responsibilities:
Program Management:
- Oversee the day-to-day operations of the Consumer Directed Services (CDS), Structured Family Caregiving (SFC) and In-Home Services (IHS) programs under Missouri Medicaid.
- Ensure that both programs are aligned with Medicaid policy and Missouri state regulations, emphasizing consumer choice and independence for Medicaid recipients.
- Develop and implement program guidelines, procedures, and protocols to improve service delivery and ensure quality care.
- Continuously monitor program performance, tracking key performance indicators (KPIs) and making adjustments as necessary.
Consumer Directed Services (CDS):
- Manage and support the CDS program, which allows Medicaid recipients to have more control over their care by hiring and managing their personal attendants or caregivers.
- Educate recipients, family members, and caregivers about the CDS program, including eligibility, benefits, and responsibilities.
- Work closely with service providers, support coordinators, and other stakeholders to ensure recipients' needs are met and services are delivered effectively.
- Resolve issues and concerns related to care management, caregiver training, and service coordination.
In-Home Services (IHS):
- Oversee the In-Home Services program, which provides home care services, including personal care assistance, homemaking, and other support to Medicaid recipients who are aging or have disabilities.
- Ensure services are delivered according to Medicaid standards and recipients' individual care plans.
- Monitor IHS provider performance to ensure that recipients receive high-quality care that meets regulatory requirements and personal needs.
- Develop and manage relationships with local agencies and service providers to facilitate IHS program coordination.
Structured Family Caregiving (SFC):
- Caregiver Training & Onboarding: Ensuring the live-in family caregiver passes background checks, completes initial orientation, and understands how to safely perform care duties (like bathing, grooming, and medication tracking)
- Conducting Monthly Progress Checks: Communicating with the family regularly—often alongside a registered nurse—via phone calls or monthly home visits to ensure the personalized care plan is being followed and the home remains
- Documentation and Service Review: ensure all required documents and services are delivered according to Medicaid standards and recipients' individual care plans.
Policy and Compliance:
- Ensure that programs comply with all state and federal regulations, including Medicaid guidelines, health and safety standards, and client rights protections.
- Regularly review and update program policies and procedures to reflect changes in Medicaid rules, state law, and federal regulations.
- Manage program audits and quality assurance initiatives to ensure that services are delivered in a compliant and ethical manner.
Budget and Financial Management:
- Assist in the development and oversight of the program budget, ensuring the efficient allocation of resources for CDS and IHS services.
- Review spending patterns and make recommendations for cost-efficiency measures while maintaining service quality.
- Work closely with the finance department to monitor program expenditures, manage invoicing, and ensure payment accuracy.
Training and Development:
- Oversee the training and professional development of service providers, caregivers, and internal staff working with the CDS and IHS programs.
- Ensure that all providers and caregivers are trained on Medicaid regulations, best practices in care, and consumer rights.
- Coordinate and facilitate training sessions, webinars, or workshops to ensure continuous skill development and compliance.
Stakeholder Engagement and Communication:
- Serve as the primary liaison between the Medicaid program, service providers, recipients, families, and other stakeholders involved in CDS and IHS.
- Address concerns and inquiries from recipients, caregivers, and external organizations.
- Promote awareness of the CDS and IHS programs within the community, ensuring that eligible individuals are informed about the available services.
- Foster strong working relationships with health agencies, advocacy groups, and other service providers.
Data Management and Reporting:
- Track and analyze data related to program performance, service delivery, recipient satisfaction, and outcomes.
- Prepare reports for senior leadership, regulatory agencies, and other stakeholders on program effectiveness, service gaps, and improvement initiatives.
- Use data-driven insights to enhance program delivery and achieve better health outcomes for recipients.
Quality Improvement:
- Implement continuous quality improvement initiatives based on feedback, audits, and performance data.
- Collaborate with internal teams to streamline processes, reduce inefficiencies, and improve client outcomes.
- Identify areas for improvement in both CDS and IHS services and work on strategic plans to address them.
