Program Coordinator/Supervisor

Divine House, Inc. is seeking a kind, compassionate and responsible individual to work as a Program Coordinator (PC) to supervise all aspects of the daily operation of a licensed group home site in Moorhead, MN.

The qualified candidate will have the ability to effectively supervise direct care staff to ensure the health, safety, and protection of rights of each person served in the program. The PC will also be responsible for overseeing the implementation of each person's served Coordinated Service and Support Plan (plan of care). The PC will ensure that they and their staff are providing the necessary support to each individual to ensure that their needs, interests, and preferences are respected and provided for in a safe and healthy environment that maximizes personal success. The PC must have the ability to communicate effectively, orally and in writing, and be able to create documents that are legible and accurate.

The PC role is a key leadership position, providing direct care support while leading by example in the home setting. The Program Coordinator is responsible for ensuring high-quality day-to-day operations — some responsibilities include medication setup and administration, meal planning and shopping, scheduling activities and appointments, addressing staffing concerns, training staff, and implementing outcomes for the individuals served. The position also works closely with Moorhead's Program Directors to maintain effective communication and ensure consistency at the program.

The schedule is flexible and could be a morning or evening PC position

A valid Driver's License is required.

Must be able to pass Minnesota Department of Human Services background study.
