Position Summary

The Program Supervisor oversees and coordinates services for children, adults, and elderly individuals with disabilities, helping them maintain independence, safety, and quality of life at home and in the community. This position ensures services are provided according to each client's approved Plan of Care and agency policies.

Key Responsibilities

• Client Care Coordination: Assist with scheduling medical appointments, follow-up visits, and annual Plan of Care reviews with case managers/support coordinators.
• Staff Supervision: Supervise Direct Support Workers (DSWs), monitor performance, ensure compliance with care plans, and address staffing concerns.
• Home Visits: Conduct weekly client home visits and additional visits as needed to monitor service quality, client safety, and satisfaction.
• Scheduling: Coordinate staff schedules, monitor attendance, arrange backup coverage, and address missed shifts.
• Hiring and Training: Assist with interviewing, hiring, onboarding, annual training, and employee evaluations.
• Documentation: Review progress notes, EVV records, timesheets, and other required documentation for accuracy and compliance.
• Safety and Compliance: Report incidents, address client concerns, protect confidentiality, and ensure compliance with agency policies and applicable regulations.
• On-Call and Shift Coverage: Participate in a rotating on-call schedule, respond to urgent staffing and client concerns, and provide direct client care or cover shifts as needed, within required training and authorization.
• Communication: Maintain professional communication with clients, families, DSWs, case managers, and agency administration.

Qualifications

• Experience in home care, disability services, or staff supervision preferred.
• Strong communication, organizational, and problem-solving skills.
• Ability to use electronic documentation and scheduling systems.
• Reliable transportation and ability to conduct home visits.
• Must meet applicable background check, training, and competency requirements.

Work Expectations

The Program Supervisor must be dependable, flexible, and available for occasional evenings, weekends, holidays, and emergencies as required by the on-call rotation or staffing needs.

salary range of $42,000–$48,000 annually for the Program Supervisor position depending on experience, qualifications, and responsibilities.
