Title: Program Therapist

Position Description The program therapist is part of the multidisciplinary team, participating in treatment team meetings to represent the therapeutic needs and goals of assigned patients. The program therapist completes psychosocial assessments on assigned geriatric, adult, and adolescent patients. Using clinical information provided through the multidisciplinary assessment process, the program therapist completes indicated treatment plans. The Program therapist meets with patients to discuss treatment plan goals, discharge criteria and plan, complete family sessions, and assess level of risk. Program therapists provide daily group therapy to geriatric, adult, and adolescent populations, as scheduled. Program therapist functions as a liaison between team members and involved social service agencies and ensures a clinically appropriate aftercare plan is in place.
