Job Summary

Responsibilities

• Conduct thorough assessments of patients’ physical conditions, including mobility, strength, and functional abilities.
• Develop and implement individualized treatment plans
• Monitor patient progress regularly, adjusting care plans as needed to maximize recovery potential
• Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards.
• Educate patients and their families on exercises, injury prevention strategies, and home care routines to support ongoing recovery

Requirements

• Valid state licensure or certification as a licensed Physical Therapist (PT).
• Proven experience practicing in various settings such as outpatient clinics, inpatient hospitals, or home health environments.
• Strong knowledge of physiology, anatomy, neurology, orthopedics, pediatrics, and geriatrics.
• Familiarity with clinical documentation standards and electronic health records (EHR/EMR systems).
• Experience working with diverse patient populations including geriatric neurological and orthopedic patients.
• Ability to develop effective treatment plans tailored to individual needs
• Excellent client communication skills to foster trust and motivate patients throughout their rehabilitation journey

Pay: $65,000.00 - $75,000.00 per year
