Physical Therapist (PT) – PRN Maple Leaf HealthCare Center Manchester, NH

$55–$60/hour Stern Therapy is seeking a PRN Physical Therapist (PT) to join our rehabilitation department. This flexible opportunity is ideal for a clinician interested in helping residents restore mobility, strength, and functional independence.

What You'll Do

• Perform physical therapy evaluations and establish individualized treatment plans
• Provide skilled interventions for mobility, balance, strength, gait, and functional performance
• Monitor resident outcomes and modify treatment plans when appropriate
• Collaborate with PTAs, nursing, and other rehabilitation professionals
• Complete accurate clinical documentation

Qualifications

• Active New Hampshire Physical Therapist license required
• Strong clinical assessment and communication skills
• Skilled nursing or post-acute experience preferred
