What they need

Help Open Doors to Second Chances.

Housing changes everything. Your work will help create the stability that makes employment, recovery, family reunification, and successful community reintegration possible. Stable housing is one of the strongest predictors of successful reentry and one of the greatest challenges facing individuals returning from incarceration.

Cornerstone Community Initiative (CCI), a new nonprofit in Monroe County, is seeking a passionate, resourceful, and relationship-driven Reentry Housing Coordinator to lead housing navigation and stabilization efforts for justice-involved individuals returning to Monroe County from incarceration.

This isn't just a housing navigation role. You'll work directly with participants, landlords, housing authorities, property managers, nonprofit partners, and community organizations to create housing opportunities, strengthen partnerships, and help individuals build stable foundations for long-term success.

Lived Experience is Valued

At Cornerstone Community Initiative, we believe lived experience strengthens our organization and helps us better serve our community.

We strongly encourage applicants with personal or familial lived experience related to incarceration, recovery, behavioral health, or navigating complex community systems to apply.

If you're passionate about housing, community collaboration, and helping people rebuild their lives, we'd love to meet you.

Job Overview

The Reentry Housing Coordinator serves as CCI's housing subject matter expert, leading efforts to increase housing access, stability, and opportunity for justice-involved individuals.

Working collaboratively with participants, landlords, housing providers, housing authorities, and community partners, you'll coordinate housing services, reduce barriers to housing stability, monitor participant progress, and support the development of current and future housing initiatives—including transitional housing.

Key Responsibilities

  • Conduct housing assessments and develop individualized housing stability plans.
  • Coordinate housing referrals, applications, placements, and follow-up services to support long-term housing success.
  • Build and maintain relationships with landlords, housing providers, housing authorities, and community partners.
  • Track housing referrals, placements, outcomes, and participant progress while maintaining accurate documentation.
  • Identify housing barriers, develop creative solutions, and strengthen community partnerships that expand housing opportunities.
  • Support the development and implementation of CCI's transitional housing and other housing initiatives.

Qualifications & Skills

We're looking for someone who is:

  • Passionate about housing stability and community reintegration.
  • An excellent relationship builder and collaborative problem solver.
  • Comfortable working with landlords, housing providers, and multidisciplinary partners.
  • Highly organized with strong documentation and case coordination skills.
  • Knowledgeable about housing resources, homelessness, supportive housing, or community-based services.
  • Committed to professionalism, accountability, dignity, and respect.

Bachelor's degree preferred. Equivalent combinations of education, professional experience, volunteer experience, and lived experience will be considered.

Experience in housing services, homelessness, supportive housing, property management, behavioral health, reentry services, or human services is highly valued.

Why Join CCI?

Cornerstone Community Initiative is building Monroe County's coordinated reentry system—connecting individuals with housing, employment, healthcare, recovery supports, education, and community resources that help them successfully rebuild their lives.

As our Reentry Housing Coordinator, you'll play a pivotal role in expanding housing opportunities, strengthening community partnerships, and helping individuals achieve one of the most important milestones in successful reentry: a safe, stable place to call home.
