Referral Coordinator Responsibilities:

• Coordinate patient transfers between acute care, sub-acute, long-term care settings and home care
• Collaborate with healthcare providers, families, and caregivers to gather patient information and develop personalized care plans
• Utilize electronic medical records and referral management systems to streamline the referral process
• Verify patient eligibility, benefits, and insurance coverage
• Communicate with patients, families, and healthcare providers to ensure seamless transitions and minimize disruptions
• Participate in quality improvement initiatives to enhance patient care and optimize clinical outcomes
• Maintain accurate records and reports, including statistical data and performance metrics

Referral Coordinator Requirements:

• Prior experience in healthcare coordination, patient access, or related field
• Strong communication, interpersonal, and problem-solving skills
• Ability to work in a fast-paced environment with multiple priorities and deadlines
• Familiarity with electronic medical records and referral management systems
• Strong attention to detail and organizational skills
