Role: RN Case Manager

Responsibilities

• Coordinate patient care across multiple locations, ensuring safe and efficient transitions through the continuum of care.
• Conduct comprehensive assessments and develop individualized care plans.
• Collaborate with physicians, therapists, social workers, and community partners to optimize patient outcomes and reduce readmissions.
• Provide patient and family education, resource referrals, and perform utilization reviews.
• Maintain thorough documentation in the EMR system.
• Engage in ongoing learning, contribute to quality improvement initiatives, and support compassionate, patient-centered care within a collaborative culture.
