Responsibilities:

• Provide weekend on-call coverage for our patients and families, responding to emergencies and supporting the care team as needed
• Conduct assessments, develop, and implement individualized care plans, and coordinate care with other healthcare professionals
• Manage pain and symptoms, providing medication and other therapeutic interventions as needed
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and coordinated support
• Participate in multidisciplinary team meetings to develop and refine patient care plans
• Maintain accurate and detailed records of patient interactions, assessments, and care provided
