Overview

La Esperanza Home Health Care Agency is seeking a compasionate and experienced Registered Nurse(RN) to join our growing home care agency in New Jersey. The RN will provide clinical oversight, perform patient assessments, develop plans of care, supervise caregivers and help ensure compliance with New Jersey home care regulations. Responsibilities

• Conduct initial and supervisory patient assessments
• Develop and update individualized plans of care
• Supervise and support CNAs and caregivers
• Complete required clinical documentation
• Coordinate with physicians patients, and families
• Ensure compliance with New Jersey regulations and agency policies
• Participate in quality assurance and staff training as needed

Qualifications

• Active New Jersey RN License
• Home Care Experience Preferred
• Strong Communication and Organizational skills
• Reliable Transportation
• CPR certification
• Bilingual (English/ Spanish) Preferred

Pay: $55.00 - $65.00 per hour
