About the role

Provide clinical care and ongoing health assessments to clients in the community. Develop and review individualised care plans. Monitor clients' health and respond appropriately to changes in condition. Coordinate care with GPs, allied health professionals, families and support workers. Provide clinical guidance and support to the care team. Promote healthy ageing, independence and quality of life. Maintain accurate clinical documentation and ensure compliance with relevant standards.

About you

• AHPRA-registered as a Registered Nurse
• Experienced in aged care, community nursing or a similar setting
• Compassionate, respectful and committed to person-centred care
• Confident working independently and as part of a multidisciplinary team
• knowledge of Aged care standards
• evidence of working rights in Australia
• Strong in clinical assessment, communication and care coordination
• Organised, reliable and committed to maintaining high professional standards
• Current driver's licence and reliable transport
• blue card and yellow card
• First aid and CPR
• NDIS workers orientation
