Responsibilities

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
• Administer medications and injections accurately, including IV infusions and catheterizations, following strict aseptic techniques and safety protocols.
• Assist with diagnostic evaluations, specimen collection/processing, and wound care procedures like suturing and dressing changes.
• Support patient mobility and comfort through activities such as tube feeding, ventilator management, and Hoyer lift operation.
• Collaborate with multidisciplinary teams in acute care settings like ICU, emergency room, or surgical units to coordinate patient care plans.
• Maintain clinical confidentiality policies while documenting medical records thoroughly and accurately in EMR/EHR systems.
• Provide specialized care for patients with complex needs including dementia, Alzheimer's disease, infectious diseases, or those requiring hospice or palliative care.

Qualifications

• Valid state licensure as a Licensed Practical Nurse (LPN).
• Proven experience working with geriatric patients in long-term care facilities, skilled nursing facilities, or assisted living environments.
• Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; familiarity with ICD-9/ICD-10 coding is preferred.
• Critical care experience in settings like ICU or trauma centers is highly desirable.
• Strong knowledge of anatomy, physiology, pharmacology, and medical terminology relevant to diverse patient populations.
• Ability to perform physical examinations, venipuncture (phlebotomy), injection administration, and sterile procedures confidently.
• Excellent communication skills for triage assessments, patient education, and interdisciplinary collaboration.
• Commitment to maintaining HIPAA compliance and adhering to long-term care regulations while delivering compassionate patient-centered care.
