We are seeking experienced and dependable Registered Nurses to join our growing team on a per diem basis.

Position Summary

The Per Diem Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes in accordance with physician orders, Medicare Conditions of Participation, and agency policies. This position requires excellent clinical judgment, strong assessment skills, and the ability to work independently while collaborating with the interdisciplinary team.

Responsibilities

• Complete skilled nursing visits, including:
• Start of Care (SOC)
• Resumption of Care (ROC)
• Recertification (RECERT)
• Routine Skilled Nursing Visits (RV)
• Discharge (DC)
• Perform comprehensive physical and psychosocial assessments.
• Develop, implement, and update individualized Plans of Care.
• Monitor patient status and identify changes in condition.
• Provide skilled nursing services including:
• Wound care and dressing changes
• Medication management and reconciliation
• Foley catheter insertion, maintenance, and care
• Ostomy care
• Diabetes management and insulin administration
• IV therapy (if qualified)
• Disease management and patient monitoring
• Educate patients and caregivers on medications, disease processes, fall prevention, infection prevention, nutrition, and home safety.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Notify physicians promptly of significant changes in patient condition and obtain new orders when appropriate.
• Complete accurate and timely documentation in the agency's electronic medical record (EMR).
• Participate in case conferences and quality improvement activities as requested.
• Maintain compliance with Medicare regulations, ACHC standards, HIPAA requirements, and agency policies.

Qualifications

• Current, unrestricted California Registered Nurse (RN) license.
• Minimum one (1) year of RN experience; home health experience preferred.
• Current CPR/BLS certification.
• Valid California driver's license, reliable vehicle, and current automobile insurance.
• Ability to travel throughout the assigned service area.
• Strong assessment, communication, organizational, and documentation skills.
• Proficiency with electronic medical records (EMR) and mobile documentation.
• Ability to work independently and manage a flexible schedule.
• Spanish speaking

Preferred Qualifications

• Previous Medicare-certified home health experience.
• OASIS documentation experience.
• Wound care experience.
• Knowledge of Medicare Conditions of Participation.
• Experience using home health EMR systems

Pay: $70.00 - $150.00 per hour

Work Location: In person
