Milwaukee & Kenosha Area | Part-Time | Per-Visit

Professional Home Care Services, Inc. is seeking experienced Registered Nurses (RNs) to provide assessment, care planning, and clinical oversight for clients receiving personal care services in their homes.

About the Position

Professional Home Care Services, Inc. hires part-time, per-visit Home Health RNs to:

• Conduct client assessments in the home.

• Develop and update individualized care plans.

• Supervise personal care workers and community-based care teams.

• Monitor clinical compliance and ensure services are provided according to the established plan of care.

• Complete electronic documentation at the point of care.

Visits are generally brief, typically lasting approximately 5–30 minutes, depending on the client's needs.

What You'll Do

• Provide one-on-one skilled nursing care and clinical oversight to clients in their homes.

• Perform comprehensive patient assessments.

• Develop care plans in collaboration with physicians and other members of the care team.

• Supervise personal care workers and provide appropriate clinical guidance.

• Complete timely and accurate electronic charting during visits.

• Manage your own schedule and travel route throughout the Milwaukee and/or Kenosha areas.

• Communicate effectively with clients, families, caregivers, physicians, and other healthcare professionals.

• Identify changes in client condition and respond appropriately.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the State of Wisconsin.

• Typically 1–2 years of clinical, home health, or community-based nursing experience.

• Strong assessment, critical-thinking, and problem-solving skills.

• Ability to work independently and manage a flexible schedule.

• Comfort using technology and willingness to learn electronic charting systems.

• Valid driver's license and reliable, insured transportation.

• Ability to travel to clients' homes throughout the assigned service area.

• Ability to pass a pre-employment physical, drug screen, and TB test.

• Ability to perform the physical requirements of the position, including standing, walking, assisting with patient ambulation and repositioning, and lifting or moving up to 50 pounds as required.

Previous home health experience is preferred but not required. Experienced nurses from other clinical settings are encouraged to apply.
