Registered Nurse Case Manager

Duties:

• Provide comprehensive nursing care to patients in various settings, including ALF, ILF,
  Memory Care, PCH, and residential homes.
• Assess patient needs and develop individualized care plans.
• Coordinate and collaborate with interdisciplinary healthcare teams to ensure continuity of care.
• Monitor patient progress and make necessary adjustments to the care plan.
• Educate patients and their families on disease progression, options of care, resources available for individuals/families facing end of life, symptom and pain management, and wound care
• Advocate for patients'rights and ensure they receive appropriate healthcare services.
• Document patient information accurately and maintain confidentiality.

Qualifications:

• Valid Registered Nurse (RN) license in the state of Texas.
• Minimum 1 year of experience as a Registered Nurse, Hospice experience is not required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a team environment.
• Proficient in electronic medical record (EMR) systems.
• Knowledge of healthcare regulations and best practices.

Pay: $70,871.00 - $85,090.00 per year
