Overview

Emmanuel Care Center - Travis are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will bring experience working as a floor nurse in skilled nursing facilities (SNF), with a strong background in behavioral health. This role involves providing high-quality nursing care to residents, ensuring their safety, comfort, and well-being through comprehensive assessments, medication management, and personalized care plans. The Registered Nurse will collaborate with interdisciplinary teams to deliver holistic patient-centered care in a dynamic environment.

Responsibilities

• Deliver direct nursing care to residents in accordance with physician orders and facility protocols.
• Monitor residents’ health status, including vital signs, mental health, and behavioral changes, documenting findings accurately.
• Administer medications and treatments safely while observing for adverse reactions or side effects.
• Assess behavioral and psychiatric needs of residents, implementing appropriate interventions and communicating with mental health professionals as needed.
• Develop, review, and update individualized care plans tailored to each resident’s physical and psychological needs.
• Collaborate effectively with physicians, therapists, social workers, and other healthcare team members to coordinate comprehensive care.
• Educate residents and their families about health conditions, medication management, and behavioral support strategies.
• Ensure compliance with all regulatory standards, safety protocols, and facility policies.
• Respond promptly to emergencies or changes in residents’ conditions, providing critical interventions when necessary.

Skills

• Proven experience as a floor nurse in a skilled nursing facility (SNF).
• Strong background in behavioral health .
• Excellent assessment skills with the ability to recognize physical and mental health issues promptly.
• Compassionate communication skills to support residents with diverse needs.
• Ability to work effectively within a multidisciplinary team in a fast-paced environment.
• Knowledge of medication administration protocols and safety procedures.
• Familiarity with regulatory compliance standards related to nursing practice in SNFs.
• Adaptability and problem-solving skills to manage complex resident cases confidently.
