Registered Nurse

This position focuses on conducting comprehensive nursing assessments that generate detailed, goal-driven care plans, ensuring each veteran receives the appropriate level of care and support. Working closely with clients, family caregivers, and the agency’s interdisciplinary team, the Field Nurse ensures that all care provided aligns with the agency’s health goals, clinical requirements, and the standards of excellence.

This individual will be responsible for assisting our clinical team with daily tasks such as:

• Conduct thorough initial and ongoing nursing assessments to identify each client’s medical, functional, and psychosocial needs.
• Develop individualized plans of care based on assessment findings.
• Communicate regularly with caregivers, physicians, and agency staff to review and update the plan of care as conditions change.
• Provide education and guidance to clients and caregivers to promote safety, independence, and health maintenance.
• Provide skilled nursing care in clients home based on physicians’ orders.
• Accurately document all assessments, care plans, and visit notes in accordance with VA and agency standards.
• Participate in interdisciplinary case conferences and contribute to care planning discussions.
• Maintain compliance with HIPAA, infection control, and all regulatory requirements.

MINIMUM REQUIRED EDUCATION:

• Graduate from an accredited nursing program
• Master's, Bachelor's, or Associate's degree (Required)

MINIMUM REQUIRED LICENSING:

• Current RN licensure as a physical in the state of Georgia

MINIMUM REQUIRED EXPERIENCE:

• 1-year previous experience as a registered nurse.
• 1 year of home health or home care experience preferred
• Effective communication skills, customer service focused, team oriented, and basic computer skills
