Registered Nurse (RN) – Initial Assessments & Care Plans

Trusted Home Care Agency is seeking a Registered Nurse (RN) on a per-diem/contract basis to conduct initial in-home client assessments and develop individualized care plans for our home care clients.

This position is ideal for an RN who is comfortable conducting independent assessments, identifying a client's care needs, and creating a clear plan of care that guides the services provided by our caregivers.

Responsibilities

• Conduct comprehensive initial in-home assessments of clients referred for home care services
• Assess the client's physical and functional status, including mobility, ADLs, personal care needs, safety, and ability to perform daily activities
• Review relevant medical history, medications, diagnoses, and current health-related needs
• Identify the client's specific needs for personal care, companionship, supervision, mobility assistance, meal preparation, medication reminders, transportation, and other authorized non-medical services
• Communicate with the client, family members, and other appropriate parties to understand the client's needs, preferences, routines, and goals
• Develop an individualized care plan based on the assessment findings
• Determine the appropriate frequency, type, and scope of home care services needed
• Clearly document assessment findings and the established plan of care
• Communicate the care plan and client-specific needs to the agency administrator and assigned caregivers
• Update or reassess the care plan when the client's needs change, as required
• Maintain accurate, timely, and confidential clinical documentation
• Follow applicable Georgia home care regulations, agency policies, and HIPAA requirements

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the State of Georgia
• Experience with home care, home health, geriatrics, community-based care, or similar settings preferred
• Strong clinical assessment and documentation skills
• Ability to independently assess clients in their homes
• Excellent communication and interpersonal skills
• Reliable transportation and willingness to travel to client homes throughout Metro Atlanta
• Professional, compassionate, and respectful approach to working with older adults and their families

Position Details

Job Type: Per Diem / Contract
Schedule: Flexible; assessments scheduled based on client referrals
Location: Client homes throughout Metro Atlanta, with a focus on Alpharetta and surrounding areas
Compensation: $55 per completed initial assessment

Most assessments are expected to take approximately one hour, depending on the client's needs.
