Registered Nurse

Job Summary:

MININUM QUALIFICATIONS

Must be 21 years of age or older

Minimum 16 hours of yearly training

First Aid certified and CPR certified

Must be able to read and write English

Excellent interpersonal skills

Bachelor’s Degree in Nursing

State of Georgia Registered Nursing License

Clean driving record

Clean Criminal Record Check

Sensitivity to the service population’s cultural and socioeconomic characteristics is required

Duties & Responsibilities

Conduct initial and annual Nursing Assessment

Provides professional growth and development of staff and ensures adequate staff education,

orientation and evaluation such as self - administration of medication, and infectious disease control.

provision of services to ensure participants health and safety.

Conducts home visits and skilled nursing services as outlined in the Service Care Plan 62, 92, 122 Home Supervisory Visits and Medication Management Visit.

Overseeing the services and supports provided to individual for general guidance in areas of compliance and quality improvement.

Supervise the performance of PCA work assignments to the individuals.

Evaluates the individual’s needs during the initial face to face visit and subsequent visits.

Identifies the appropriate staff to provide the care needed.

Develops and revising individual Service Plan of Care as appropriate.

Reviews the contents of the Service Plan at each visit to ensure compliance with regulatory agencies and internal quality controls. Completes clinical record reviews, nursing notes, and documentation of supervisory visits.

At least once annually, assist in completing an in home supervisory visit to observe the in-home performance of PCAs.

Supervises any services provided for medically fragile clients, and defines those types of clients appropriately.

Conducts in service training for staff.

Report any problems or progress to the Director of Nursing.
