Responsibilities

• Provide direct patient care in a home environment, including assessment, treatment, and education.
• Develop and implement individualized care plans in collaboration with healthcare providers.
• Monitor patient progress and communicate changes to the care team.
• Maintain accurate and timely documentation of patient care activities.
• Educate patients and families on health management and disease prevention.
• Ensure compliance with all regulatory and company policies.

Requirements

• Current Registered Nurse (RN) license in the state of Michigan.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver’s license OR reliable transportation for home visits.
