Breakaway Toowoomba Inc. (Breakaway) has been supporting people with disability across the Toowoomba region since 1986. What started as a small, family-based respite service has grown into a trusted local provider delivering a wide range of supports, including independent living, respite, community participation, in-home support, and day services.

With over 130 local staff, our focus remains firmly on delivering high-quality, person-centred care while continuing to invest in our people, systems, and infrastructure. Our vision is to be a leading provider in disability and human services, delivering reliable, connected, and quality support that empowers clients and their families.

At Breakaway, you are not just joining a team - you are becoming part of a purpose-driven organisation committed to inclusion, innovation, and continuous improvement.

About the Role

We are seeking a dedicated Registered Nurse to support clients with chronic and complex disability and health needs within our Specialist Disability Accommodation (SDA) and community settings.

Working alongside a supportive team of Nurses and Disability Support Workers, you will play a key role in assessing, planning, delivering, and evaluating client care. You will collaborate closely with the Clinical Lead to ensure high-quality, compliant, and person-centred outcomes.

Key Responsibilities

  • Deliver safe and effective nursing care aligned with individual care plans and NDIS requirements
  • Monitor and respond to changes in client health and wellbeing
  • Support clients with medication management, enteral feeding, personal care, and community participation
  • Coordinate appointments and assist clients to achieve their goals and independence
  • Maintain accurate clinical documentation, including progress notes and incident reports
  • Provide clinical guidance and support to Disability Support Workers
  • Ensure compliance with NMBA standards, NDIS Practice Standards, and organisational policies
  • Participate in quality audits, risk management, and continuous improvement initiatives

What We’re Looking For

Essential:

  • Current and unrestricted AHPRA registration
  • Strong clinical skills and care planning experience
  • Experience supporting clients with complex health needs
  • Excellent communication and interpersonal skills
  • Strong organisational and time management abilities
  • Ability to work collaboratively within a multidisciplinary team
  • Current Blue Card and NDIS Worker Screening Clearance (or willingness to obtain)
  • Current Driver Licence and CPR Certificate

Highly Desirable:

  • Experience in paediatric complex care
  • Experience in disability or community-based care
  • Enteral feeding, medication administration, and complex care experience
  • Knowledge of aspiration risk, respiratory support, or oxygen therapy
  • Experience working with families and multidisciplinary teams

Why Choose Breakaway?

  • Above-award rates of pay
  • Meaningful, purpose-driven work
  • Supportive leadership and collaborative team culture
  • Opportunities for professional development and career growth
  • Salary packaging and employee wellbeing initiatives
