Home Health Field Clinician

Position Summary

We are seeking a dedicated, compassionate, and professional healthcare clinician to join our Home Health team. This position involves providing skilled nursing services to patients in the comfort of their own homes, ensuring high-quality, patient-centered care in accordance with physician orders, individualized plans of care, and agency policies.

The clinician will be responsible for traveling to patients' residences to perform assessments, provide skilled nursing interventions, monitor patient conditions, educate patients and caregivers, and complete all required clinical documentation accurately and in a timely manner.

Key Responsibilities

• Conduct home visits according to assigned patient schedules and care plans.
• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and routine skilled nursing visits as assigned and within professional scope.
• Complete comprehensive patient assessments and required documentation.
• Provide medication management, disease process education, wound care, and other skilled nursing services as ordered.
• Communicate changes in patient conditions to physicians and the clinical management team.
• Coordinate patient care with other healthcare professionals and agency staff.
• Maintain accurate and timely clinical documentation in accordance with agency policies and applicable regulations.
• Promote patient safety, independence, and quality of life.

Compensation Structure

This position offers a per-visit compensation structure, with different payment rates depending on the type and complexity of the visit.

Payment categories include:

• Start of Care (SOC): Separate rate for initial comprehensive admission assessments.
• Resumption of Care (ROC): Designated rate for resuming services following hospitalization or interruption of care.
• Recertification Visits: Specific rate for reassessment and continuation of home health services.
• Discharge Visits: Separate rate for completing discharge assessments and documentation.
• Regular Skilled Nursing Visits: Standard per-visit rate for routine patient care.
• Other Specialized Visits: Compensation determined according to the type of service provided.

Specific compensation rates will be discussed during the interview and provided in accordance with the agency's established payment schedule and applicable employment requirements.

Qualifications

• Active and unrestricted RN or LPN license, as appropriate for assigned duties.
• Previous home health experience preferred.
• Strong assessment, communication, and clinical documentation skills.
• Ability to work independently and manage assigned patient visits.
• Reliable transportation and willingness to travel to patients' homes.
• Ability to maintain professional relationships with patients, families, physicians, and agency personnel.

This is a field-based position requiring travel to patients' homes within the agency's designated service area. Visit assignments may vary depending on patient needs, clinician availability, and geographic location.
