Job Type: PRN / Per Diem
Pay: Per Visit

• Start of Care (SOC): $200
• Recertification: $160
• Resumption of Care (ROC): $150
• Discharge (DC): $100
• Supervisory Visit: $50
• Follow-Up Visit: $60

River Falls Home Health is a locally owned and operated agency dedicated to delivering high-quality, compassionate care to patients in the comfort of their own homes. We are currently expanding and looking for skilled Registered Nurses (RNs) to join our dynamic team on a per-diem basis.

Responsibilities:

• Conduct patient Start of Care (SOC) assessments
• Complete Resumption of Care (ROC), Recertifications, and Discharges
• Provide follow-up skilled nursing visits in accordance with physician orders and care plans
• Perform supervisory visits for home health aides
• Document care and assessments
• Collaborate with the clinical team to ensure the best outcomes for patients
• Educate patients and caregivers on disease processes, medications, and care plans

Requirements:

• Current RN license in the state of Idaho
• Minimum of 1 year of clinical nursing experience (Home Health experience preferred)
• Reliable transportation, valid driver's license, and proof of insurance
• Excellent communication, time management, and documentation skills
• Ability to work independently in the field
