About the role

The RN Case Manager plans, organizes, and directs hospice care for patients and families in their homes and communities. The nurse coordinates individualized care, focusing on comfort, symptom management, patient dignity, and family support.

Key responsibilities

• Complete initial and ongoing comprehensive patient and family assessments.
• Develop, implement, and regularly update individualized plans of care.
• Manage an assigned hospice patient caseload and coordinate care with the interdisciplinary team.
• Administer prescribed medications and treatments, with an emphasis on pain and symptom management.
• Communicate with physicians regarding changes in patient condition and obtain necessary orders.
• Educate patients, families, and caregivers regarding medications, symptoms, and end-of-life care.
• Maintain accurate, timely clinical documentation.
• Coordinate medical equipment, supplies, and other necessary services.
• Supervise hospice aides and delegate responsibilities appropriately.
• Participate in interdisciplinary group (IDG) meetings and quality improvement activities.
• Participate in on-call duties according to company policy.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in Ohio.
• Current CPR certification.
• Preferred: 1–2 years of acute care experience.
• Preferred: 1–2 years of home care experience.
• Bachelor's degree and one year of hospice experience preferred.
• Strong clinical assessment, communication, and critical-thinking skills.
• Valid driver's license and reliable, insured vehicle.
• Ability to perform the physical requirements of patient care.
