Registered Nurse

Center for Transitional Living (CTL) is seeking a compassionate, dependable Registered Nurse (RN) to provide in-home nursing assessments and oversight for individuals enrolled in Connecticut's Adult Family Living (AFL) program.

This is a flexible, field-based position ideal for an experienced RN looking for supplemental income and the ability to manage their own schedule.

The primary responsibility is conducting one-hour, in-person home visits approximately every 60 days to assess client well-being, monitor care needs, and ensure appropriate services are being provided.

Key Responsibilities

• Conduct in-person nursing visits at clients' homes every 60 days.
• Complete nursing assessments and monitor changes in clients' health and functional status.
• Evaluate caregiver performance and the client's overall safety and well-being.
• Review medications, medical conditions, and changes in care needs.
• Identify potential health or safety concerns and communicate findings to the care management team.
• Complete required nursing documentation accurately and promptly.
• Coordinate with caregivers, families, and care managers as needed.
• Maintain compliance with applicable Connecticut DSS and AFL program requirements.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in Connecticut.
• Experience in home care, community nursing, geriatrics, or Medicaid waiver programs preferred.
• Familiarity with Connecticut's Adult Family Living (AFL) or CHCPE programs is a plus.
• Strong assessment, documentation, and communication skills.
• Reliable transportation and willingness to travel to clients' homes.
• Ability to work independently and manage scheduled visits.
