Job Summary

We are seeking a dedicated and compassionate Registered Nurse Part time/ PRN, that can work to join our dynamic personal care team here at Hope and Dreams Care Solutions LLC. Hope and Dreams Care Solutions provide non-medical home health personal care to clients in there home. As the role of a RN, you will oversee clinical safety, manage the care plan documentation.

Responsibilities

• Conduct comprehensive in-home clinical assessments of the clients physical,emotional and environmental needs.
• Evaluates the the clients activities of daily living (ADLs) to determine the exact level of support needed.
• Writes and updates the individualized plan of care that ensures what task the PCA is authorized to perform. And up date and report any changes with in care plan.
• Coordinates with the client primary care provider to align the care plan with medical orders.
• Direct Supervision and Field Evaluations Periodically visits the clients home or evaluates performance through required supervisory checks to observe the PCA's work and ensure they are following safety and quality standards.
• Evaluates the PCA's competence in performing daily living assistance, safe transfers, and proper hygiene support, providing guidance or additional training when needed.
• Valid RN license with current registration in the state of practice.
• Background Check
• CPR First Aide

Pay: $38.00 - $40.00 per hour
