Nurse (RN/LPN) – Home Health

Position Summary

Avanzar Home Health Care is seeking compassionate and dependable Skilled Nurses to provide high-quality in-home patient care. Nurses will assess patient needs, develop and implement care plans, administer treatments, and collaborate with physicians and interdisciplinary healthcare teams to ensure excellent patient outcomes in the comfort of the patient’s home.

Responsibilities

• Perform comprehensive patient assessments and ongoing evaluations
• Administer medications, injections, wound care, IV therapy, and other skilled nursing services
• Monitor and document patient condition, progress, and response to treatment
• Develop and update individualized care plans
• Educate patients and family members on disease management, medications, and home care procedures
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals
• Maintain accurate and timely clinical documentation in compliance with state and federal regulations
• Ensure patient safety and infection control standards are followed
• Respond promptly to patient concerns and changes in condition

Qualifications

• Current and active RN or LPN license in the state of employment
• Minimum of 1 year nursing experience preferred; home health experience a plus
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively
• Current CPR certification
• Valid Fingerprint card
• Valid driver’s license, reliable transportation, and auto insurance
