Registered Nurse – Clinical Manager / Director of Nursing

Position Summary

Rescue Heart Medical Services is seeking a Registered Nurse to serve as Clinical Manager / Director of Nursing for our new home health agency. This is a founding clinical leadership role: you will be named as our Clinical Manager on state licensure, Medicare enrollment, and accreditation applications, and you will provide real clinical oversight as the agency launches and grows.

About the Opportunity

This role is for an RN who wants to help build a home health agency from the ground up. Rescue Heart is moving through the licensing and certification process, and we need an experienced nurse leader whose credentials and clinical judgment will anchor that process.

You will shape our clinical policies, set the standard of care, and lead the nursing team as we begin admitting patients. If you have home health experience and want a leadership seat at a growing organization, we want to talk with you.

Key Responsibilities

Startup and licensure phase

• Serve as the designated Clinical Manager / Director of Nursing on state licensure, Medicare (CMS-855A) enrollment, and accreditation applications (e.g., CHAP, ACHC, or The Joint Commission)
• Develop and approve clinical policies and procedures, including admissions, plans of care, infection control, medication management, and emergency preparedness
• Help prepare for and participate in state licensing surveys and accreditation site visits
• Build the clinical documentation framework, including OASIS assessment workflows and plan-of-care templates
• Help establish the Quality Assessment and Performance Improvement (QAPI) program

Operational phase

• Provide ongoing clinical oversight of all patient care delivered by the agency
• Conduct or supervise start-of-care assessments, OASIS assessments, and recertifications
• Develop, review, and coordinate individualized plans of care with physicians and allied providers
• Supervise, train, and evaluate RNs, LPNs, home health aides, and therapy staff; conduct required aide supervisory visits
• Verify staff competencies, licenses, and in-service training requirements
• Review clinical documentation for accuracy, timeliness, and regulatory compliance
• Lead case conferences and coordinate care across disciplines
• Monitor patient outcomes, incidents, complaints, and infection-control data through QAPI
• Ensure compliance with Medicare Conditions of Participation, state regulations, and HIPAA
• Provide direct patient care visits as census and staffing require

Required Qualifications

• Active, unrestricted Registered Nurse license in [New Jersey] (or a multistate compact license valid in [New Jersey])
• Minimum of 2 years of RN experience, including at least 1 year in home health care
• Working knowledge of Medicare home health Conditions of Participation and OASIS documentation
• Experience supervising or delegating to licensed and unlicensed nursing staff
• Current CPR/BLS certification
• Willingness to be named as Clinical Manager / Director of Nursing on licensure, enrollment, and accreditation applications
• Ability to pass a criminal background check and federal and state exclusion-list screening (OIG/SAM)
• Valid driver’s license, reliable transportation, and auto insurance for home visits

Preferred Qualifications

• Bachelor of Science in Nursing (BSN) or higher
• Prior experience as a Director of Nursing, Clinical Manager, or Supervising Nurse in home health
• Experience helping a new agency through licensure, Medicare certification, or accreditation surveys
• OASIS certification (COS-C or equivalent) and familiarity with PDGM billing
• Experience with home health EMR systems (e.g., Axxess, WellSky, HomeCare HomeBase)
• Home health or case management certification

Credential Use and Commitment

By accepting this position, the selected candidate agrees to the following:

• Rescue Heart may list the candidate’s name, RN license number, and qualifications as Clinical Manager / Director of Nursing on state licensure, Medicare enrollment, and accreditation applications
• The candidate will actively perform the clinical oversight duties described above; this is a working role, not a name-only designation
• The candidate will participate in licensing surveys, accreditation visits, and related interviews
• The candidate will notify Rescue Heart immediately of any change in license status, disciplinary action, or exclusion
• The candidate will give at least [60] days’ written notice before resigning, so the agency can appoint a qualified replacement and notify regulators as required

These terms will be confirmed in a written agreement at the time of hire.

Pay: $41.79 - $50.33 per hour
