Job Summary

Join our nursing team as a Registered Nurse (RN) and deliver attentive, evidence-informed care across patient needs and care settings. You’ll assess patients, coordinate treatment, administer medications, and document care accurately while protecting privacy and following infection-control practices. Your impact: Timely assessment and coordinated care help patients and families make informed decisions and support safer transitions between care settings.

Duties

• Assess patient condition, obtain vital signs, support diagnostic evaluation, and communicate changes to the care team.
• Administer medications, injections, and IV infusion; perform IV insertion, catheter care, wound care, and other procedures within your scope and training.
• Provide patient care in long-term care setting.
• Maintain medical documentation in electronic health record (EHR) or electronic medical record (EMR) systems; follow HIPAA privacy requirements and applicable documentation standards.
• Support triage, patient education, health coaching, case management, discharge planning, and collaboration with patients, families, and clinicians.

Experience

• Minimum of 1 year of RN experience (Highly preferred; will consider exceptional new graduates with strong clinical rotations in long-term care).
• Experience in a Skilled Nursing (SNF), Long-Term Care (LTC), or sub-acute rehabilitation setting is strongly preferred.
• Familiarity with PCC (PointClickCare) or equivalent Electronic Medical Record (EMR) software.
• Geriatric nursing expertise, including managing complex, chronic comorbidities.
• Proficiency in sub-acute clinical skills, including wound care, IV therapy, medication administration, and tracheostomy care.
• Strong assessment skills to detect subtle changes in resident status and prevent unnecessary hospital readmissions.
• Proven leadership abilities to effectively supervise, delegate to, and support LPNs and CNAs.
• Deep understanding of State and Federal regulations (CMS guidelines) governing long-term care facilities.
• Exceptional communication skills for collaborating with physicians, interdisciplinary teams, residents, and families.

Pay: $38.00 - $40.00 per hour
