Life Plus is looking for an experienced New Zealand Registered Nurse to join our friendly, supportive team as a Registered Nurse / Complex Care Consultant.

You’ll work closely with clients, whānau, care teams and health professionals to coordinate safe, high-quality, person-centred care in the community.
You will:

• Complete clinical assessments and develop individualised care plans
• Coordinate care with clients, whānau and multidisciplinary teams
• Monitor client progress and complete regular clinical reviews
• Provide clinical guidance, training and competency support to care staff
• Liaise with GPs, specialists, ACC, NASC and allied health professionals
• Provide calm, confident clinical advice when complex situations arise
• Build trusted relationships with clients, whānau, referrers and the wider Life Plus team

About you You’ll be a New Zealand Registered Nurse with a current APC and New Zealand nursing experience. You’ll enjoy working independently, connecting with people and finding practical solutions that improve clients’ lives.
