Registered Nurse – Infusion – Community Based

Job Summary

Join as a Home Infusion RN in a community-based role focused on delivering expert infusion therapy, building patient relationships, and providing care in patients’ homes and community settings.

• Provide individualized infusion treatments, including vascular access and medication administration, ensuring patient safety and comfort.
• Coordinate referrals, verify clinical and administrative details, and develop personalized care plans based on patient assessments.
• Educate patients and caregivers on therapy protocols, equipment use, and vascular access care.
• Monitor therapy progress, adjust care plans, and communicate with interdisciplinary teams for optimal outcomes.
• Travel within the service area using a company-provided vehicle, with no weekend or holiday work and minimal on-call responsibilities.
• Supervise and support Licensed Practical Nurses, ensuring adherence to care standards.
• Maintain accurate documentation, ensure compliance with clinical protocols, and participate in quality improvement initiatives.

Qualifications & Skills

• Minimum of three years RN experience required.
• Home infusion and home care experience preferred.
• Strong clinical assessment, patient education, and communication skills.
• Ability to work independently and collaboratively in a fast-paced environment.
